24-Hour Availability
All of our office locations are open Monday through Friday, 8:00AM until 5:00PM. Our group offers physician availability 24 hours a day, 7 days a week. By dialing their Family Physicians Group office, patients can communicate with the physician on call for guidance. Evening hours and Saturday hours are available at select offices. Inquire as to which offices are participating, because extended office hours may change.
ACCESS Sites
ACCESS (Automated Community Connection Economic Self-Sufficiency) Florida has opened ACCESS Sites in the community. These sites will provide quick an easy means to apply for public assistance regarding Medicaid, food stamps and cash assistance.

At-Home Care
By initiating home care follow-up to the chronically ill, we provide for more patient-physician interaction, in addition to reducing patients’ reliance on emergency room visits and unnecessary hospitalizations. The doctor is able to develop a deep knowledge of that patient’s living conditions, like health status, home environment, family dynamics, finances, behavioral issues and medication education.

Behavioral Health
Our Behavioral Health program is designed to integrate your medical care and behavioral health needs through consultation, evaluation and by identifying treatment options. Patients who suffer from depression, bipolar disorder, substance abuse and schizophrenia are provided easy access to psychologists, psychiatrists and therapists (in select offices), as well as pharmacy and transportation services.

Community Outreach
The Family Physicians Group staff hosts health fairs and educational seminars on various topics, including nutrition, diabetes, Medicare Advantage Plans and more. We regularly provide free health screenings and relevant educational materials to those who participate. Contact the Marketing Department (1-866-999-3741) for upcoming events. Our helpful staff of customer service representatives is available to take your call, Mondays through Fridays, 8:00AM to 5:00PM. Comprehensive Care
Case Managers assist the physician in developing and following a comprehensive care plan. As licensed, registered nurses with extensive experience with industry practices, perform, among other things, daily hospital tracking; total care and follow-up from the clinic to hospital to nursing home to the patient’s home; and education on Advance Directives.
Diagnostics & Procedures
In-house diagnostics and procedures include lab work, x-rays, ultrasounds, Bone Density Scans (DEXA), Nerve Conduction Studies (NCS), Electrocardiograms (EKGs), echocardiograms (‘echoes’), pulse oximetry, Pulmonary Function Tests (PFTs), Intravenous therapy (IVs), Nebulizer treatments and others.
Disease Management
Our full-scale Disease Management program monitors and controls chronic illnesses, such as diabetes, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and End Stage Renal Failure at no additional cost. Enrollees receive vouchers to help cover the cost of co-payments for select medications and medical devices. Our registered nurses conduct daily outreach phone calls to patients in an effort to improve patients’ compliance to medical treatment and overall quality of life.

Eligible patients receive free diabetic and Chronic Obstructive Pulmonary Disease (COPD) supplies, like glucose monitors, strips and lancets, as well as other costly injectibles at a discounted price. Certain restrictions apply.
Hospital/Nursing Home Care
Family Physicians Group’s very own hospitalist group, Physicians Hospitalist Partners (PHP), effectively monitor patients’ care at the hospital and nursing home level. PHP’s seven full-time physicians have privileges in major hospitals in Orange, Seminole, and Osceola counties, including Orlando Regional Health System (ORHS), Florida Hospital, Osceola Regional and Health Central. They function as a communication bridge for Primary Care Physicians to effectively monitor patients after they are discharged from the hospital.
Pharmacy
Family Physicians Group serves patients with the convenience of an internal personalized pharmacy to deliver cost-effective medications to our patients. Family Physicians Pharmacy (FPRx) provides free shipping and handling, bilingual customer service and flexible payment options.

Non-emergency round-trip services are offered free of charge to patients enrolled in a Medicare Advantage Plan. Certain restrictions apply. Complete and submit applications at our office and receive transportation in as soon as three weeks.
